To start the death claim process, please complete this online form. Or contact our Service Center at 309-558-3077 or 800-447-9811, Monday through Friday, 8 a.m. to 4:30 p.m. (Central).

 

You can review or update your information by logging in to your online member account.

There are three ways to change your beneficiary:

  1. Log in to your online member account at member.modernwoodmen.org.
  2. Contact your Modern Woodmen representative. You can find your local office at reps.modernwoodmen.org.
  3. Complete the beneficiary change form and send it to our headquarters:
    1. By mail: P.O. Box 2005, Rock Island, Illinois 61204-2005.
    2. By fax: 309-793-5547.
